Can a caller tell if you have heard their voice mail?

Interesting, I mean there is a “delivered” tag on iMessage and I think there was once a “read” tag, I think !
I cannot think how anyone would know if you had listened to a message unless the app is set up to reply when a message is opened.

There is nothing I am aware of that allows this to be done.

I don't think anyone would know. You can always tap on it, but instead of playing it, tap the share arrow and save it to yourself iMessage. Then listen to it.
