Doing a google search, it looks like this is from an app you may have downloaded from the app store that allows you to make custom icons. Does that ring any bells?

Ya I did download a app which customizes my icons but I deleted the app and don't use it I can delete it than right

It's same like creating a shortcut in desktop PC. But this particular shortcuts in iOS need internet to launch and always leave a browser window open in the background.
