iPhone 6 Camera - Hardware Issue or Software Issue?

I currently have a iPhone 6 64GB (AT&T). I went to a Wedding this past Saturday and took tons of photos. i used the "Mono" Filter. I'm aware of how to ficus a camera and /or camera phone so as I took the photo it was CRISP. The next day I went through the photos and noticed they were ALL out of focus until i hit the "Edit" button and then they magically refocused but by choosing cancel they were fuzzy again. So I choose to Revert the photos and my focus problem disappeared.

This is an odd issue to me, is anyone else experiencing this issue? This sounds like a Software issue more than a hardware issue.
