iPhoneCFA--APP that reappears after reboot...iPhone 5

I have this APP/icon on my screen. It says iPhoneCFA and has default "no icon" icon. If I click it, it appears to do nothing, blanks screen then returns to home screen that it is on.
I am able to delete, but it comes back with reboot.
Any ideas? I have performed the standard LMGTFY and nothing.

Have you tried a hard reboot? That usually took care of it for me when I had this issue. Another thought would be connecting to iTunes and deleting it from there.
