how can I restore my iPhone 5s back to factory settings?

I want to give it to my church to give to soldiers who need phones and I need to get my personal information off it first.

Navigate to the settings app on your device. Go under general once you are in there. All the way at the bottom of the general settings is "reset." Click on that and choose Erase All Content and Settings. The device will ask you several questions to confirm this is in fact what you want to do, but once you are done you are good to go!
