Why is my apple mobile display is getting overlap

Am using 5s with 9.2 OS having issue in contact option. While creating a new contact when we push the lock screen button & unlock the phone, dial pad screen is getting overlap with the add to contact screen.

Can you show a screen shot of this? Also, try a hard reset.
Hold the top or side power/sleep button and the home button at the same time until the Apple logo appears, then release.
