Notes app gone from my phone

I just tried to access my notes app on my 4S Iphone and it is gone!! I had several exsisting notes that had important information. Is there a way to retrieve and how do I get the app back on my screen I had some issues this weekend with text messaging and phone screen freeze did this cause the problem...

I've never known a native app to disappear. Type 'notes' in your spotlight, it should tell you where it is. Also, add pictures of your home screens here if that doesn't work.
