I have recently discovered a bunch of sites with questionable content on my router accesed through my wife's iPhone 8. She said she did not visit those sites. Anyway, long story short, is it possible that someone was remotely using her iPhone, and if yes, would I see such activity on our home router? She claims her account/phone was hacked. Thank you.

Anything is possible but highly unlikely that someone is visiting sites thru your device. Most hacks try to access your data

Does anyone else have access to your wife’s iPhone

It’s possible. I mean MAC cloning is still a thing, I suppose. Is your Wi-Fi network secure? To be on the safe side, change the passcode associated with it. Disable guest access, if enabled.

Possible? Yes. Likely? Plausible.

But as @kataran said, most hackers want your data, not use your device to listen to Asian music.

I mean, I turn off wifi and use LTE when I want to listen to Asian music.

Hmmmm that's an awful lot of trouble just to surf porn. Just to be on the safe side change your WiFi password, your router password, your wife's Apple ID password, and enable two factor authentication https://support.apple.com/en-us/HT204915. Now if it still happens, get a divorce your wifes lying to you ...
