Why am I unable to add member to family sharing?

unable to add member to family sharing

I sent the invitation, it was received, the member accepted, but gets the message 'Failed accepting invitation.' We can't figure out what we are doing wrong. Suggestions?

How many family members? 6 is the limit. Maybe these will help.
