I forgot my Apple ID and Password and Email address.

My phone was disabled and need to have that information to use it again. The pass code was incorrectly entered many times.I can't reset it since its asking for the Apple ID and I can't remember it anymore.

Time to make a new apple ID. You do not really have any option unless someone at apple over the phone will give that information out but I doubt it. how many email addresses do you have? you can always submit a request for your username and password if you remember your email. I dont know anyone that has that many email addresses they couldnt guess it.
