Help please, what is the best app for Patient charts

I'm a physician and I want to have all my patient chart information on my iPhone , what is the best app for this?

Are the charts on a server? For example, does someone enter all the data to a computer that you could sync the iPhone with? I don't know of an app that deals specifically with charts, but perhaps you could sync them as notes? I'm really not sure, and probably not much help.
