Continuous heart rate monitor?

Does anyone know if the Apple Watch continuously monitors heart rate, or only when in workout mode? Thanks.

The way I read the material available it seems to me they (Apple) are saying the heart rate monitor works when using an app on the device that uses the data... during a workout, with the app monitoring, or the Heartbeat Glance.

That's my read too. Just found it hard to believe.

So ... HealthKit will have periodic spikes of heart rate data?
