Can someone identify this app and any nefarious ways it could be used.

On my girlfriends phone but she is unaware how it got there and suspects an outside source installed it. I can add a screenshot if someone can tell me how.

I'm not sure what that is. What's the app called? It doesn't look legit and I'd recommend removing it if she's not aware of how it got there.

Dial #3001#12345#* and tap Call to enter Field Test Mode
*#21# and tap Call for the Setting interrogation for call forwards.
