touch screen replaced, but phone disabled and I forgot my Apple ID, what can I do?

I dropped my phone and when I rebooted the 3,6 and 9 digits on the touchscreen were inop. I tried to restart, but still inop. The phone was "disabled" due to the number of attempts to restart. I had the touchscreen replaced, but in order to "enable" the phone, I must have my Apple ID and password. I have the latter but not the ID. What can I do?
