My phone is getting very warm and doesn't hold a charge for long, any ideas?

The phone is approximately 3 years old and I carry it in my pocket except when it is on the charger.

Li-ion batteries are supposed to last at least 1000 cycles before it loses capacity. With 3 years and charging everyday, (=365x3) I'd say your battery is due to be replaced. It's best to contact your local Apple Store to have it tested.
