How do I keep my iCloud keychain to stay turned off on 5S?

icloud keychain won't stay turned off on 5S

followed the simple directions to turn icloud keychain off, but it won't stay off - keeps toggling back on, showing "pending" after going back to settings menu. The phone is repeatedly asking me to approve with security code or another device.
