Are the phone and watch paired? Reboot the watch and the phone... Un-pair then re-pair the watch and phone if the issue continues.

If your iPhone is not on the lock screen, meaning that you're looking at it unlocked, your Apple watch will not register phone calls or notifications. It makes sense because if you're looking at your phone what you need to watch to buzz

Do you have wifi calling enabled? My watch won't ring when wifi calling is enabled (connected to wifi). If I turn off wifi calling it's fine.

Might be worth giving it a shot.
