Why does my iPhone volume keep adjusting itself?

So as of a few nights ago, I discovered that my headphones volume was randomly being lowered, when I wasn't pressing the button to do so. It does this with and without the case on, so I know it isn't that. It happens whenever I have apps open or closed, no matter what I am doing, the volume will lower itself until it is completely muted. I've tried turning it on and off, doing a restart, and a factory reset. This is an extremely annoying issue, and I know it isn't my headphones, as I tested them on a friends phone. If you have any help, please respond.

Go to Settings -> Music -> Volume Limit, and then make the necessary adjustments to see if that fixes the problem.

If you adjust the volume while listening to music, it may get it off where you had it for calls. 3 places to adjust volume.
1. Side of phone
2. Pull up Control Center
3. Settings >Sounds
