What is running in "background" mean?

What does "Background" mean (on the Battery Usage screen, Location access screen and in iOS world in general)?

a) the app is on screen but the screen is off

or

b) the app is running but not on screen: I can double-press the home button and scroll tot he app

or

c) the app is closed: it is not accessible through double-tapping the home button.
