Sound issues iphone 4s

I have perfect sound during calls, but serious sound distortion and echo on video playback and people cannot hear me on facetime, just background noise?

On Facetime I have heard some abnormal noises before and as far as video goes it could depend if it was a home made or a professional video and the volume. Have you checked in your sound settings to see if you have EQ on or off? Try that and see if it makes a difference on whether you have it on or off. If on try and turn it off to see if that makes a difference.
