Where do I enter the promo code?

In iTunes on your computer, you can navigate to iTunes Store > App Store. There should be a Redeem link in the App Store Quick Links on the right.

Or open the App Store app on your iPhone, navigate to the Featured tab (leftmost tab at the bottom), scroll to the bottom of the page and there should be a Redeem button there as well.
