Is there a way I can find out where/how an unknown user accessed my FaceTime account?

I had a notification on my phone yesterday that someone was using my phone number /name to access facetime on a new I-Pad. Is there a way I can find out where/how this happened? What devices are associated with my phone number/name?

Nope. I can only assume that a loved one or an ex has logged into to FaceTime using your credentials. What I suggest you do, is change your current passcode to a new one that only YOU know. By doing so, the other person will not be able to use FaceTime with your credentials.
