I doubt it's hacked. You probably have a rogue app installed. Still, if you're overly concerned about it, make a Genius Bar appointment with the nearest Apple Store.

I would just delete the few apps you recently installed to see if it helps. If that doesn't I would re-install your os.

Are you on ios9?
Ios 9 can be jailbroken (hacked) just by visiting a website or reviving a text message. I recommend to restore completely your iPhone and set it up as new.
