You would need to call AppleCare at 1-800-275-2273 and they can assist from there.

You should have a secondary rescue email address set up for this occasion (it's in your Apple account settings). That's the email you need to get in to.
Most, if not all, email accounts have provisions for when you forget your password/login information. Follow those guides for retrieval.
