1. Logout of iCloud
2. Disable the Find My iPhone setting.
3. Logout of the iTunes & App Store.
4. Disable iMessage and FaceTime
5. Logout of GameCenter.
6. Logout of/disassociate all apps in which you are logged in; Facebook, Twitter, Dropbox, Evernote, etc.

Afterwards, go to Settings - General - Reset and then select "Erase All Contents and Settings".
