Call Verizon Wireless if you have insurance of some sort on the device from Verizon. If you purchased AppleCare Plus through Verizon then great. Call or take your iPhone 6 to the nearest Apple Retail Store.

You contact the insurance provider. Asurion. Phoneclaim.com – Verizon phones replaced fast! File a claim for your lost, stolen or damaged phone
You can start on the Verizon website if you are more comfortable doing that. https://totalmobileprotection.verizonwireless.com/public/prepare-for-claim
But you will end up having your claim processed by Asurion.
