Deleted photo reappears for split second upon unlocking iPad air

I have this weird thing that happens, when I unlock my iPad Air, a split second image of a deleted photo appears, doesn't happen every time, intermittently when you least expect it. Sometimes when you go to your Photos as well. Not sure why this does this because it has been deleted as well in the "recently deleted" folder. My photos do not get sent to iCloud, it is turned off. Any ideas? Thanks

I used to see deleted photos behind png images on my camera roll thumbnail grid view. I think iOS 8 stopped it. Never on the lock screen though. Was it ever used as a wallpaper?
