My wife’s (Oticon) don’t have a microphone that transmits back to the phone, when she takes a call she still has to talk into the mic on the phone while the audio comes through her HA
Do yours transmit your speech? That’d be the only way I think. Get an Apple Watch ;-)
I just did and it worked and repeated that it was going to use the speaker. I looked at the settings for phone and in accessibility and everything looks default there
Mine is also set to silent but if I press the large button twice, slowly, from a dark face it will do that chirp. They have to be far enough apart to not trigger Apple Pay
Maybe that's what's happening? Sometimes I notice that I have been working on something with my hand pronated flexed...

Love them, I use them all day long at home and work for podcasts, music and phone calls. Best headphones ever
I find it's the way I hold them putting them back in. If I cover the sensors they think they're back in my ears. I use that to start the podcast back up as I put one away
I use Siri on my watch for everything. Messaging, nav, reminders, grocery list, HomeKit, weather, timers, alarms, music podcasts
It's slow when my phone isn't nearby but it's very handy
