Unlock not Working... Right Method?

I have had an untethered Jailbreak for a while.

Trying to implement the unlock here: Jailbroken iPhone 4S Users Can Now SIM Unlock Their Phones to Work on Different Carriers.
-However the IMSI in SIM usually matches the IMSI in SAM.
-The iphone doesn't activate when I plug it in for the first time.
-It's also unclear when the original sim card should be in and when the new sim card should be in.
-Is this unlock still valid?

Also tried to install the ultrasn0w apps via instructions here: Unlock iOS 5.1.1 on Your Jailbroken iPhone 4 / iPhone 3GS / iPhone 4S
-But I'm receiving an error during application install that doesn't let me get to the unlock process.
