I don't know the name of the slider but the name of the theme is SECEDE LS and you can find it at mac themes. Hope that helps.

Everytime I come in here I get depressed LOL I'm still on 4.3 waiting for the damn JB and I've sold my iPad so I'm in Team Puregatory for now. I'm hoping a 4.3 JB comes out soon so I can join the ranks again.

I can't even imagine the withdrawal pains you must be experiencing.

I thought you were thinking about going back to 4.2.X because the 4.3 jailbreak was taking too long. I just can't picture you without a jailbroken iPhone or iPad.
