iTunes Not Syncing Playlist Changes?

So I've looked at my setting in iTunes & can't figure out what's going on?

If I add a couple of songs to a playlist on my iPhone, the next time I sync it to iTunes it won't keep/ save the changes I made on the Playlist on device.

I'm sure it's something really stupid, anyone care to educate me?

Are the changes still being saved on iTunes? Is your iPhone being recognized on iTunes? If so, the syncing problem can be fixed by doing a hard reset on your iPhone. Yes I know its a shame to have to go back to factory settings but in order to keep your playlist up to date that is the best recommendation I can think of. Of course, I'm offering this suggestion because I'm assuming you've turned the phone off and back on right?

What I *think* is happening is that the old playlist saved in iTunes is being saved back into the iPhone thus erasing the new changes to the playlists made on the phone.
