This has been happening for a while and it's really quite annoying. It doesn't happen often (maybe a couple of times per week) but it'll play a little sound effect then skip to the next track. Firmware is up to date and battery is always almost full. Any idea what's going wrong?

I no longer have an iPod, but during the time I did have it, I don't recall it ever skipping to the next track. By the way, which model iPod do you have?

I've had this issue several times with my iPod nano. Specially when earphones are plugged via cord. There seems to be a misconection of some sort, it evens tells the name the name of the song aloud before skipping the track. Particularly when on the move, either working out at the gym or running. I've noticed it doesn't happen when using Bluetooth earphones/speakers
