What app are you using to get the icons to be placed where you want them?

It’s not an app, it’s a website that you upload your wall to and then choose which icons you want “blanked out” and save them to home screen. After you save them you place them where they go, like doing a puzzle. I moved the one icon out of place just to show you what it looks like.
