A app appeared on phone during the night. Not sure how it was requested.

I have never heard of an just appearing. Are you on family plan? Or liked to other ios devices?

That should have read I have never seen an app just appear. Are you on family plan or linked to any other IOS devices?

Maybe it's not an app. Is it possible you might have saved a safari web link to your home screen? I've done this before by mistake, while thinking I was bookmarking a page.
