You enter the passcode when prompted. If you are already signed in to the iTunes & App Store, open the app, located the app you're trying to install, and then select to install it. Depending on how you have your device configured, it may or may not prompt you for a passcode.

The same password you use for your Apple ID most likely. Unless you set up separate credentials for iTunes.
