not an app as such but if you use safari and go to iphone.tvcatchup.com (i can't post a link) and it will give you a channel list and you can choose which channel to watch. It's about 30 seconds behind "live" tv so isn't too bad.

You can add an icon to your homepage which will take you straight to that page

You need to get a slingbox.

Sorry, didn't realise you were looking for something worldwide - tvcatchup is uk based and streams all the freeview channels

What about MobileTV via Cydia. Its mostly used for Tv shows and Movies...
