3 people out of millions does not usually equate to a widespread software issue, in my opinion. You mentioned in a previous post, "I noticed in the logs that there are thousands of pairing requests per day from some "headset" when I'm at home. It appears a neighbor of mine has a headset in pairing mode." That could indeed be the problem. I think you also stated that you did not use Bluetooth a lot. If that's true, turn the Bluetooth feature off until you're ready to pair it with one of your Bluetooth products.
I understand 3 out of a million isn't a widespread issue, but it indicates a problem effecting more than one person.
As for turning off Bluetooth, since iOS so heavily used it for its own features (Beacons, Continuity, Handover, etc) that's not an ideal situation.
