You don’t lose anything, at all.
You just can’t use an other Apple ID for the next 90 days to redownload something you already downloaded in the past.

It gives you the right to redownload stuff that you purchased / downloaded with an Apple ID.
If you change to a different Apple ID, you’ll have to wait 90 days before you can change again.
