I have iphone 4s on which I have some games already being played. Now I bought iphone 5+ so I want to transfer the games to new phone. Can it be done if I login via same game center ID?

You first need to back up your 4s and then sign in to your new iPhone using your same Apple ID and restore your back up and pick up where you left off
