It's a combination. The icons are a mixture of Symbian Belle (Cydia) and Purple Orbs (iPhantom I think but her site is not working for me right now) and the widget is called MinimalBottom Time4 - also from iPhantom.

I may still have the zip files where I emailed them to myself since iPhantom is now a private site.
