Seriously!? After 4.1 I had no issues with the proximity sensor issue, I felt it was resolved. BUT past few days I have had the worse problem with it and its pissing me off. It actually seems worse then it was before. UGH. Anyone else with this problem..again?

i never had till this night i was on the phone and i could see the screen flickering in and out
my fix?
rotate your hand slightly so that your hand faces more to the back of your head (not a lot just slightly )
this fixed it for me at least during that phone call

The sensor on i4 is above the ear speaker, unlike previous iphones which had it to the left. Try moving the phone down a little bit when on a call. If that doesn't do it and you're already on 4.1, then you probably have a hardware problem and should exchange the phone.

Would having a screen protector on be an issue in that working correctly?

I could possibly be an issue, especially those privacy ones. There's a LED that emits a light, and the sensor detects whether the light is reflected back. If the screen protector blocks the reflected light, it could interfere with the sensor.
