Out of warranty replacement they will charge you the subsidized price. It was $299 for the 16GB 3G, might be cheaper now that the new models are out. Call Apple or go to a store and ask.
Screen protectors protect against scratches, not a 6ft fall to concrete. I should have had my case on, I KNOW!, but it was dirty and I hadn't been able to clean it after I got my new-er phone.

If I were you, I would spend a little extra money and take it to Apple. Sure they are more expensive, but you never know what would happen if you fix it yourself, or if you bring it to someone else to fix.
If you are doing it yourself, be careful with following:
1. Gasket, be careful to not damage the Gasket while opening.
2. Connector # 3, if you break the connector # 3 on the Logic Board, then your iPhone is brick.
3. LCD, be careful while removing the LCD, almost everyone breaks the LCD.
