What causes the Episode icon to be blank, see pictures attached.

All my Movies & TV Shows are all similar meaning the information like Title, Year, Episode Number, etc. but for some reason I get blank icons in some of my library.

Like this one...the gray with TV icon.



This what I want all of them to appear in my Air...

I've had that same damn problem going back to iOS 6 which happens to be when I started to buy TV Shows off of iTunes.

Whether the episode was fully downloaded or not.
