Hi,

My phone was unlocked and had firmware 3.01, but iTune was requesting the original operator SIM to operate. I updated to 3.12 (7D11) and unlocked again with black1rain RC3. Same issue iTune is still requesting the original operator SIM. I can not activate the iPhone.

Have any of you encountered this problem? Any suggestion?

You haven't unlocked with blackra1n. You have just jailbreaked the iPhone. So to unlock you need to install Sn0w(blacksn0w).
Blackra1n should have hacktivated the iPhone. If none works, then restore and remember to set it up as new.
