How to change iMessage - Message ID

My daughter has a 4S, and we recently had to replace it through Verizon due to overheating. I backed up her old phone to iCloud, and then on the replacement I restored from iCloud. We have her in our contacts list by her first name, but now when she responds to one of our texts, or sends an original text, the ID is showing her e-mail address instead of her name. It is doing this with everyone that she sends a text to. What could be the problem?

Go to Settings-Messages-Send & Receive and change it to her phone number.

Go to Settings-Messages-Send & Receive and change it to her phone number.

Her phone number is grayed out, and won't let her check it. Check is next to email address.

Remove the email address from the list so it will default to her phone number.
