how do I set up outlook 2013 on windows 7 to work with iCloud?

I am using an HP Elitebook with Windows 7 and Office 365. I want to access my iCloud mail/calendar/contacts.

I have 2 factor authentication on iCloud. I get my unique authentication key phrase but Outlook continues to ask for the password on the smtp server after the imap server accepts the key phrase
