We are looking for an app that can help score participants in a story telling contest, though this could help scoring for anything. This is what we envision:

In the app you enter in the name of the event, all the participants names, and the scoring ranges, like 1-5 or 1-10. When done the app emails you a PDF which you can print. The PDF will look like one of those multiple choice test-taking cards from school, but may have special registration marks around the edges. You would print a copy for everyone who shows up to score (the audience, judges, etc).

At the event, the audience or judges fill in the 1-5 scoring circles for each participant as the event goes on. When it's done you collect all the sheets, stack them, and then use the app, which uses the iPhone's camera, to take a picture of each sheet, like using a QR code reader. The app uses the special registration marks to find the circles, and tallies the participants sheet by sheet.

When you click done the app displays the participants in high-score order.

Anyone know of an app that can do something like that?

That is an extremely specific app description. IMHO you could accomplish this task with a piece of paper and a pencil and no technology is needed.

Not so easy if you have 100 people in the audience scoring 12 people. The time to tally all of the numbers takes way too long.

Have you checked Google Docs? There may be a way with that.
