Why is my ‘ear’ speaker quiet

When making calls the top speaker(that you hold to your ear) is extremely quiet: have to make calls on speakerphone.

Also when video recording, the sound pick up is delayed by around 5 seconds

If the phone is an iPhone, make a Genius Bar appointment with the nearest Apple Store to have it checked out. That is my recommendation.

I’d definitely take it to the Genius Bar at a nearby Apple Store or contact Apple Support.

Is it dirty? Have you tried cleaning it with a dry toothbrush?

Agreed. I ‘brush’ my phone every other week and it works wonders.

Assuming you tried to raise the earpiece volume while in a call with no success then follow the appointment advice

That's the old code sequence for updating the PRL (Preferred Roaming List) on old CDMA devices. It doesn't apply to LTE devices. And doesn't have anything to do with fixing sound issues unfortunately even on those phones.
