Your best bet is to take the phone in to your local Apple store for either an exchange (if it's within the 1 year warranty), or a repair. Do not have the phone repaired by anyone other than an official Apple repair store... that will void your warranty if it is still in place.

Contact your service provider to ensure that your device is correctly setup/provisioned on their network and also to ensure that there are not tower problems in your area.
