Make a Genius Bar appointment with the nearest Apple Store.

What model iPhone are you using? This issue generally points to a battery that has reached the end of it's usable life and the power is draining from the device faster than the phone can recharge it. Apple batteries are designed to maintain 80% capacity for 500 full charge cycles, so if you're using an older model it could just be time for an upgrade. If you're using a newer device or you're covered by AppleCare then head over to an Apple Store.
